FORM 1095-C
In December 2024, federal legislation was signed that provided some updates to the Affordable Care Act. Among the updates, Form 1095-C is NO LONGER REQUIRED to be submitted with your federal tax filing. The update changed reporting requirements to reduce unnecessary paperwork for individuals and employers.
Under these updated rules, employers and health plans are no longer required to automatically provide Form 1095-C to all members.
However, you may still request this document if you feel like you need to have it. Once your request is received via one of the following methods, your Form 1095-C will be provided within 30 days. We strongly recommend that you submit your request to the email address below, to have that documentation trail.
